3. Appointments & Telehealth

By booking an appointment (in-clinic or telehealth), you agree to:

• Provide accurate personal and health information
• Attend your appointment at the scheduled time
• Pay any applicable fees

Telehealth is only available for clinically appropriate matters and may require in-person follow-up.

4. Fees & Billing

MydocX operates as a mixed-billing clinic.

Fees may include:

• Out-of-pocket gap payments
• Private telehealth fees
• Fees for certificates, referrals, reports and procedures

5. Emergencies

Our clinic does not provide emergency services. For emergencies, call Triple Zero (000) immediately.

6. Patient Responsibilities

Patients must:

• Be truthful and respectful
• Follow clinical instructions
• Use telehealth appropriately
• Not misuse prescriptions or medical certificates

We reserve the right to refuse service where necessary.

7. Privacy

Your personal information is protected under the Privacy Act and handled in accordance with our Privacy Policy.

8. Limitation of Liability

MydocX is not responsible for:

• Delays in telehealth connections
• Issues arising from inaccurate information provided by the patient
• Services outside our control
